Book an Online Consultation

If you need medical advice, support for a new or existing condition, or guidance on symptoms, you can submit an online consultation to your GP practice. Your request will be reviewed by a member of the clinical team, who will decide the most appropriate next step, such as a message reply, phone call, video call, or face-to-face appointment.

Fit Note (Sick Note) Request

If you need a Fit Note for work due to illness or injury, you can request one online. This includes extensions to an existing Fit Note. You may be asked to provide supporting information to help your clinician assess your request.

Please note: We are unable to provide a sick note for periods of absence lasting for less than 7 days. In these circumstances you should self certify.
Is the note required due to admission to hospital or are you regularly seeing the hospital about the condition that is making you poorly?
Please Note: If you are under the hospital for regular review or have been recently discharged because of the condition making you unable to work, it is the hospital specialists responsibility for issuing a fit note. This is made very clear in government guidance. They may tell you that the GP will do it, but it is their responsibility to do it as they are the ones with the detailed knowledge of your condition and best placed to provide the detail on the certificate. Key messages are: You must ask for a certificate whilst in clinic OR You must ask for a certificate before you leave the hospital Often the hospital will not offer one without being asked You will need to go back to the hospital if you have failed to do this

What dates do you require on the Fit Note? Please note: Doctors are not permitted to issue backdated notes which pre-date your first assessment for the condition. Take the example of a patient who suffers from an illness and misses 2 weeks of work. They only make contact with the surgery at the end of that period to obtain a fit note. Unfortunately the doctor cannot issue a backdated note to cover the second week of absence. You do not need a return to work note, even if you plan to return before the originally stated end date on the certificate.
